How do I create an SQL input adapter?

The SQL input adapter type has been removed. The same functionality can now be achieved by creating an execute step with a schedule. The SQL configuration is placed in the execute step.

To reuse SQL configuration a settings item can be created and be referenced by multiple execute steps.
